A comprehensive look at and review of your current medications including monitoring, reconciliation, and administration.

  • $250 one-time fee for assessment and explaining medications (up to 2 hours)

Recurring Fees:

  • $125/hr*
  • Management of 0-5 pills included
  • Management of 6-10 pills additional $25
  • Management of 10+ pills additional $50
  • Will work with a sliding scale fee on a case-by-case basis.

Resources

We have several resources that have been recommended and used by our clients in the past that are Senior friendly and ones we would trust with our own family members. Some of them are Estate planners or attorney’s, visiting physician’s, Senior real estate specialist, Medicare specialists. Downsizers, organizers, mover’s financial planners and many more.

We also have our own group of care giver specialists that are trained and ready to assist you and your senior loved one. 

ADVOCACY

Kimberly in her 25 years of being a Nurse has gained the experience and expertise that is needed to be the voice of a client/family that is unfamiliar with what to expect when it is needed. She can be present on phone calls or in person as a local representative for a client/family. This can be bedside in a Hospital/Rehab setting or requesting a meeting with a community where you are not feeling heard and or to explain to you the rights you have as a client/family member in the healthcare environment.

Caregiving

Kimberly has a large group of experienced caregivers both male and females that are ready to serve you and your Senior loved one. These caregivers are independent which means the terms of your relationship with your in-home worker are in your control. CMA has also invested in a HIPAA compliant app called Serenity engage that allows you to be in contact with your loved ones and their care team in real time. This provides an extra layer of reassurance and communication to provide additional reassurance in the care being provided.

We also have our own group of care giver specialists that are trained and ready to assist you and your senior loved one. 

Nursing

Kimberly has 25 years of hands-on experience as a Registered Nurse, leader, and a healthcare professional. She can provide private duty Nursing services such as but not limited too; wound care, antibiotic administration, medication management, catheter changes, trach and respiratory support.

Senior Housing

Let Kimberly meet with you in person, do an assessment and help determine what your best options would be for housing/relocation. Kimberly will take into consideration your physical, mental, psychosocial and financial situation when making recommendations. She will not only recommend but then take you to tour your options in person or virtually in order to make the right choice the first time. Every time we move a senior there will be a bit of a decline as well as a transitional phase which can takes weeks to months. We do not want to have to do this more than once to an individual if at all possible.

Refer to Kimberly’s hub of trusted resources who she and others in the Senior industry count on to make this transition as seamless as possible.

Placement-FREE SERVICE

At Care Matters Always, we believe in the importance of a seamless transition to a new home. As part of our comprehensive placement services, we now include FREE caregiving hours during this crucial period. Our dedicated caregivers offer the support needed to ensure a comfortable and successful adjustment.

Kimberly has been in the Denver area for over 6 years and is familiar with the Senior Living options out there. Also, the leadership changes and staffing challenges that COVID has caused. The recommendations made are those that are deemed to be the best for your taking into consideration the many factors that contribute to this ONE TIME MOVE.

She can also bring on board her caregiving staff to help while at home and then at your chosen community to better help with this so very important transition. There would be an hourly fee for the caregivers.

Long-Term Care

Whether it’s Medicaid, long-term care, or nursing home placement, Kimberly and the team at Care Matters Always will help you. Not sure which exactly you need? We’re here to help guide you every step of the way! Based on the amount of assistance you need, these services typically cost $500-750.

Case Management

If you are local or not there is a lot to keep track of when dealing with an aging loved one. Kimberly or a qualified member of her team can take on the role of providing and leading the collaborative process that facilitates recommended treatment plans to assure the appropriate medical care is provided to your senior loved one. We are educators that help their patients/families understand their options concerning the specific situation they are dealing with at the time. We act as a liaison between patients and their treatment or care options.

Dementia Care Consulting

Kimberly Diaz, certified in PAC (Positive Approach to Care) Dementia Care Consulting and Training, empowers caregivers, communities and families with strategies for effective communication and care. Her approach focuses on practical changes and preparation for the dementia journey, enhancing confidence and quality of life for both caregivers and those with dementia.

At Care Matters Always, we are focused on giving individuals control over their end-of-life choices. We help you understand your options, connect you with planning experts, lawyers, and provide information on the Medical Aid In Dying (MAID) program.
